Design Details
The ballot layout is deliberately simple so guests can fill them out quickly without missing a category. Each ballot fits in one hand and takes under a minute to complete. The categories were chosen to give multiple costume styles a path to victory, because the person in the $300 store-bought costume and the person wearing a cardboard box painted to look like a coffin both deserve a shot. The gothic styling keeps the ballots on-theme without making them hard to read in dim lighting.
Print multiple sheets (plan for one ballot per guest per round if you want to run elimination voting). Cut along the perforation marks. Collect completed ballots in a cauldron or decorative box.
